Great article ... thanks for posting! Incidents like these are the genesis of many of the rules we've embedded in our standards and processes. Unfortunately, those are the things some appear to be in a rush to throw out because of their implications for cost and schedule - we can do it faster, better, and cheaper if we don't bother with pesky things like last-minute borescope inspections. Always nice to revisit the "why" behind the rules!
